What can I expect from Advance Care Planning?

Advance care planning allows you to take control of your future medical care, ensuring your wishes are known and respected while providing peace of mind for both you and your loved ones. It’s normal to want to put off - or even avoid - conversations about your wishes and what you’d want if you were unable to speak for yourself - including who you’d want to speak for you. Together we can consider what is most important to you, what you do - or don’t - want to have happen and what you’d want your loved ones and decision makers to know. We will approach this conversation gently, compassionately and thoughtfully. If you want, we can also review, or complete, your states’ legal forms for stating your wishes including an advance directive and medical orders (or physician’s orders) for life sustaining treatment (MOLST & POLST forms).
